Derbyshire Community Health Services NHS Foundation Trust (DCHS) is an inclusive organisation of some 4,500 staff, making us one of the largest providers of specialist community health services in the country, serving a patient population of more than one million.
Diversity of talent:
DCHS is an organisation that embraces the innovation, talent and diversity of thought that exists in Derbyshire and we will always push boundaries and challenge ourselves to create an environment that improves outcomes for our communities.
We recognise that the diversity of our people and their talents help us deliver high quality and safe care. It makes us who we are. This helps us to further develop our workforce culture, increase staff engagement and improve clinical outcomes.
We want to become an even better place to work valuing everyone’s contribution because everyone matters. We want to be an exemplar of best equalities practice.
Vision and values:
Our vision is to be the best provider of local healthcare and to be a great place to work. We will do this by bringing to life our values of:
- Getting the basics right
- Acting with compassion and respect
- Making a difference
- Valuing and developing teamwork
- Valuing everyone's contribution because everyone matters
The DCHS Way:
Rated Outstanding by the Care Quality Commission (CQC), we provide personalised and safe community-based health services that enrich the lives of local people and communities.
We do this by bringing to life “the DCHS Way”, a fundamental pledge to our staff and patients which promises - in simple terms - how we will govern and manage our organisation:
Our Quadruple aim:
“Simultaneously improving the health of the population, enhancing the experience and outcomes of the patient, improving staff experience and reducing the per capita cost of care for the benefit of communities”
Our quadruple aim is our clinical strategy that is aligned with Joined Up Care Derbyshire partners and with national policy to improve the lives of people who live in Derbyshire.
Joined Up Care Derbyshire:
DCHS plays a very active role in the county’s Integrated Care System (ICS), Joined Up Care Derbyshire. The ICS brings together health and social care organisations across Derbyshire. Working together more closely than ever before, the ambition is to provide the best care and services for people and make them as efficient and effective as possible.
The plans set out a vision for a more joined-up approach to health and social care, the steps that should be taken to get there and how everyone involved needs to work together to improve what we deliver.
